This letter is to inform you that your employment with Wizard Industries Ltd. has been terminated for poor performance effective from 1^st October 2016.

As regards to your appraisal for the last two quarters, your performance has been ranked as below average. Since you were warned the first time and had shown no visible effort in improving your performance, we have been compelled to come to this decision.

We will send you the relevant papers and request you to sign them and send them back to us. On the completion of this process, you will receive the leaving salary, medical and insurance pay due till 1^st October 2016. We thank you for your service and wish you the best for your future.
